What is the 245D Waiver?

The 245D Waiver is part of Minnesota’s Home and Community-Based Services (HCBS) program. It allows individuals with disabilities to receive the support they need to live safely and independently in their homes or communities, rather than in institutional settings.

At Adam’s Place, we are a licensed 245D provider, committed to delivering high-quality, person-centered care under this waiver program.

You may qualify for 245D Waiver services if you:

  • Are diagnosed with a disability that requires support.

  • Are eligible for Medical Assistance (Medicaid)

  • Have a case manager who coordinates your services.

  • Need supports to live independently or participate in the community.
