Our 245D Home and Community-Based Services help individuals with disabilities live as independently as possible while receiving the support they need.
245D Services
The 245D Waiver is part of Minnesota’s Home and Community-Based Services (HCBS) program. It allows individuals with disabilities to receive the support they need to live safely and independently in their homes or communities, rather than in institutional settings.

Recuperative Services
Our Recuperative Care program offers a safe place for individuals recovering from illness, injury, or surgery who do not have stable housing.
